Long-term disability insurance (LTD) replaces a portion of your income if you are unable to work due to illness or injury (see Covered Events below). This page describes the optional Long-Term Disability insurance plan, MedPlus Advantage Program, sponsored by the American Medical Association (AMA). This program is available to UW Residents and Fellows, and the policy plan year runs from July 1 to June 30.
As a state employee you also have the option of enrolling in this program or the optional PEBB long-term disability insurance program available to all faculty and staff. When considering long-term disability insurance you should consider which program would be best for you before you make an enrollment decision.
You are eligible if you are a M.D., Ph.D., Psy.D, D.O., D.M.D., or D.D.S. who is a full-time Chief Resident, Chief Resident non-ACGME, Resident, Fellow Senior Fellow, or Senior Fellow Trainee enrolled in one of the academic departments at the University of Washington School of Medicine, or are enrolled in the School of Dentistry, School of Nursing, or School of Pharmacy.
Note: There is a 12-month waiting period for pre-existing conditions evident 180 days prior to enrolling.
|Total Disability||$2,500 flat per month for the remainder of your life or until your total disability ends.|
|Permanent and Total Disability||$5,000 monthly for first 12 months. At 12 months the prognosis must be reconfirmed. Once reconfirmed, there is a $5,000 monthly payment for the remainder of your life or until your total disability ends, plus a one-time lump sum payment of $5,000 after 12 months of Permanent and Total Disability.|
|Additional Income Benefit Reduction||Total Disability: If your insurance payment plus other income would exceed $5,000, your insurance payment is reduced so that your total income does not exceed $5,000.|
|Permanent and Total Disability: If your insurance plus other income would exceed $10,000, your insurance payment is reduced so that your total income does not exceed $10,000.|
|Loan Payoff Benefit||Repayment of up to $225,000 of eligible debt.|

To enroll, complete the Resident & Fellows LTD online enrollment form.
Claims are filed by contacting the UW Benefits Office at 206-543-2800 or firstname.lastname@example.org.
If you wish to cancel your LTD coverage, please send a hard copy letter to the UW Benefits Office indicating your wish to terminate coverage. Please include your Employee Identification Number (EID) or Social Security number and signature on the letter. Your LTD benefit coverage will terminate the end of the month in which UW Benefits receives the notification.
You can continue to protect your income as you enter practice by converting to an individual policy. You must apply for either continuation of coverage (within 31 days after the date your AMA benefit terminates) or conversion of coverage (within 60 days after the date your AMA benefit terminates).

If you apply after the deadlines stated above, you will not receive discounted rates, and you will be required to provide personal medical information and complete a medical exam to determine your insurability.
If you will be continuing employment with the UW following your Residency or Fellowship, you must contact the UW Benefits Office to ask for your payroll deductions to stop. Notify us of your appointment change by sending an email to firstname.lastname@example.org.
