Life insurance is available regardless of health issues, as long as the eligible employee applies for coverage during the first 60 days of initial insurance eligibility. For amounts above the "guarantee issue" limits, and for all life insurance requested after the initial 60-day window, eligible employees are required to provide evidence of insurability (EOI).
The University pays Basic Life Insurance and AD&D premiums at no cost to you. Other coverage options allow you to design the rest of your plan. Study the chart below to determine which parts of the program suit your needs.

If you apply later than 60 days from the date you become eligible or no later than 60 days from your marriage or the effective date of your Washington State-registered domestic partnership, you may purchase up to $50,000 of coverage without providing ROI. After 60 days, or for coverage in excess of $50,000, you must provide evidence of your spouse's or QDP's insurability at your expense. |

The UW pays for $25,000 of life insurance on each eligible employee. If you go on leave or layoff status and wish to continue optional life insurance on yourself, you will be required to pay the UW's contribution of Basic Life and AD&D insurance during that period.
An employee pays $0.50 per family, regardless of the number of dependents.
