Purpose

This is a guideline for filling out the name and address field on our Refund Check Request form UoW 1768. Name and address is limited to 5 lines.

Personal Titles and Formalities

Do not include titles such as Manager, President, M.D., or Phd. with names.
Do not include formalities such as Mr., Mrs., Miss, Ms, Dr., or Professor with names.

Do not put two names as payee unless you connect them with an "and" or an "or" (see examples below). If we receive a request with two names but no and or or we will return it to the issuing department.

There must be either a first name, a first name with a middle initial, or a first initial with a middle name (i.e. John D. Smith, J. Doe Smith are Ok but J.D. Smith is not acceptable).

Do not use initials, acronyms or non-standard abbreviations in the name (i.e. Dept of Social & Health Svc is Ok but DSHS is not).
