In our pursuit of continued process improvement, we have made significant revisions to online Annual Report of Outside Activities (formerly Form 1461).
The system allows faculty, librarians and academic staff to build reports and access report data any time via the web using an approved browser. In addition, we have added the ability to copy activities from previous year reports to help save you time. We have carefully tested the system changes and are confident that it will better serve everyone.
Please understand that all reporting must occur online and we can not accept paper based forms. While we have taken efforts to ensure that the process is intuitive, we strongly encourage everyone to read the FAQs listed below, and/or print the Annual Report Tutorial. If you still have difficulty using the system after reading the tutorial, please contact Academic Human Resources.
Please note that even if you do not have Activities to Report, we still ask that you use the system to create and submit a report with no activities.

Microsoft ceased further development and bug fixes of Internet Explorer on the Macintosh in June, 2003. This browser is essentially out of date and will never be improved. Users should use one of the browsers listed below. Despite a lack of official approval from the system vendor, we have had heard of no problems from users using Mozilla Firefox on a Macintosh.
Basically, you just create a new annual year report, and then submit it without adding any activities. See below for a list of steps:

Each member of the faculty, librarians, and other academic personnel shall submit a summary of all outside professional work, both for remuneration and for public or community service, for the previous academic year, to the dean, director of University Libraries, or vice president, with a copy to the Provost, by November 15 of each year. The University of Washington has developed Form UoW 1461, Summary of Outside Professional Services, to facilitate this reporting.
Yes! This year, we added the ability to copy activities from previous academic year reports. Because this system was launched in October 2004, individuals will only be able to copy activities from reports that have been completed using this system. Most people will be able to copy activities from their 2003-2004 academic year report. See below for specific steps:
No. You may have added all of your activities to your report, but you must still submit the report, just as you would mail a paper form. This is how Academic Human Resources knows you are finished reporting activities. To finish your report, see the steps below:
