Annual Report of Outside Activities


General Information


Dear University of Washington Faculty, Librarians and Academic Staff,

In our pursuit of continued process improvement, we have made significant revisions to online Annual Report of Outside Activities (formerly Form 1461).

The system allows faculty, librarians and academic staff to build reports and access report data any time via the web using an approved browser. In addition, we have added the ability to copy activities from previous year reports to help save you time. We have carefully tested the system changes and are confident that it will better serve everyone.

Please understand that all reporting must occur online and we can not accept paper based forms. While we have taken efforts to ensure that the process is intuitive, we strongly encourage everyone to read the FAQs listed below, and/or print the Annual Report Tutorial. If you still have difficulty using the system after reading the tutorial, please contact Academic Human Resources.

Please note that even if you do not have Activities to Report, we still ask that you use the system to create and submit a report with no activities.

Frequently Asked Questions



Why won't Internet Explorer on my Mac work with the system?


Microsoft ceased further development and bug fixes of Internet Explorer on the Macintosh in June, 2003. This browser is essentially out of date and will never be improved. Users should use one of the browsers listed below. Despite a lack of official approval from the system vendor, we have had heard of no problems from users using Mozilla Firefox on a Macintosh.


How do I submit my report if I have have no activities to report?


Basically, you just create a new annual year report, and then submit it without adding any activities. See below for a list of steps:

Create A New Academic Year Report:

  1. Navigate to the Outside Activities page either by:
    1. Clicking Outside Activities in the upper left side of the screen, or
    2. Clicking My Home in the upper right side of the screen, and then clicking Outside Activities (1461) on the left side of the screen.
  2. Click Begin Yearly Report on the left side of the screen.
  3. Select the appropriate academic year. Activity reporting, whether compensated or uncompensated, follows the university academic year (July 1 - June 30).
  4. Click Finish on the right side of the screen. This will create a new academic year, under which new activities can be reported.

    Submit your Report with No Activities:
  5. Click SUBMIT REPORT from the left side of the page. This will submit a report with no activities.

Why do I have to complete an Annual Report of Outside Activities every year?

Each member of the faculty, librarians, and other academic personnel shall submit a summary of all outside professional work, both for remuneration and for public or community service, for the previous academic year, to the dean, director of University Libraries, or vice president, with a copy to the Provost, by November 15 of each year. The University of Washington has developed Form UoW 1461, Summary of Outside Professional Services, to facilitate this reporting.


I participate in an activity every year. Can I copy information from last year's report?


Yes! This year, we added the ability to copy activities from previous academic year reports. Because this system was launched in October 2004, individuals will only be able to copy activities from reports that have been completed using this system. Most people will be able to copy activities from their 2003-2004 academic year report. See below for specific steps:

  1. Select the Academic Year Report where you want the copied activities to appear by:
    1. Selecting an existing academic year report (see tutorial), or
    2. Creating a new academic year report (see tutorial).
  2. Click COPY ACTIVITY from the left side of the screen.
  3. Select the activity you want to copy from the pulldown menu.
  4. Click OK on the right side of the screen.
  5. The copied activity will appear in your activity list of the report. Note: the copied activity is not yet reported. You must complete Steps 7-8.
  6. Click the name of the copied activity from the right side of the screen. You can now review a summary of your activity.
  7. If the information is CORRECT:
    1. Click COMPLETE from the left side of the screen.
    If the information is NOT CORRECT:
    1. Click the EDIT THIS ACTIVITY on the left side of the screen.
    2. Fill in or update the required information.
    3. Select Continue from the right side of the screen.
    4. Fill or update the required information.
    5. Select Finish from the right side of the screen. Note: the next step must be completed to submit the activity. If you do not complete the activity now, you can always edit and/or submit it later.
    6. You can now review a summary of your activity. If the information is correct, click COMPLETE from the left side of the screen.

My report is listed in Ongoing Preparation. Am I done yet?


No. You may have added all of your activities to your report, but you must still submit the report, just as you would mail a paper form. This is how Academic Human Resources knows you are finished reporting activities. To finish your report, see the steps below:

  1. Click on the academic year of the report you want to submit.
  2. Click SUBMIT REPORT from the left side of the page.

I would like to keep a printed copy of my report. Can I print my report?


Yes! This year, we added the ability to print a summary of individual activities in each report. See the steps below:

  1. Select an existing academic year report.
  2. Select an existing activity from the list on the right side of the screen.
  3. Click Click here for a Printer-Friendly view of this Activity
  4. Print the page.
  5. Click OK to return to the activity overview page.
  6. Click GO BACK from the left side of the screen to view other activities in the current academic year report. Repeat Steps 2-4 to print other activities in the current report
